Please refer practices and researchers, not patients. Our preparations are supplied for research use only and are restricted to licensed physicians and qualified facilities — if a patient is looking for care, point them to their own licensed provider, who can contact us directly.
We ask you to confirm you have your colleague’s permission before sharing their details, and we contact you before we contact them, so no one is approached cold in your name. Do not include any patient information in this form. What we collect and how it is handled is set out in the Privacy Notice.
